In order to enroll and join Intracoastal Cremation Society,
we have a simple four step process for you to complete.

1.  Print the Membership Application or for added convenience, submit online.

2.  Print and complete the Authorization for Cremation form

3.  If you chose to submit your membership application online
and did not pay your dues online, click here to do so or you can print all the
necessary forms above, complete by hand, and mail a check to the address below.


All printed forms and check payment should be mailed to:

(If you elected to complete your membership application and payment of fees online, please disregard item #1
and proceed to item #2.  Item #2 "Authorization for Cremation" will need to be mailed to the address listed above.
